advancing the goals of our organization. It’s a great & fun way to serve PACR
and network with others. Please consider nominating a colleague or yourself to

permission to
the Nominating
Committee to
place my name on
the ballot for

aforementioned
office for
calendar year
2010. I also
proclaim, that
if elected, I
will maintain my
present status
as an active
member in good
standing for the
calendar year
2011 and will
accept all
duties of the
office as
outlined in the
“Pennsylvania
Association of
Cancer
Registrars
Operating Manual
for the
Executive
Committee.” I
understand that
a forfeit or a
resignation of
membership will
result in a loss
of the office.

of each year to
be considered
eligible for
that year’s
ballot. The
Nominating
Committee

right to

eligibility of
